Option 1
Insert the extension foam mattress at the foot end, and stays in place when adjustments are made to the bed.

Option 2
Attach the lower leg rest extension(Extension foam mattress not included) to the mattress base without tools, which moves along with any adjustments and thus allows the leg elevation position.
